The Home Care Specialist is a limited term, grant funded position (anticipated to last for at least one-year). The position works as part of the site team to enhance housing stability and quality of life for some of our most vulnerable residents. In this role, the Specialist will observe, create recommendations, and develop action plans. They will actively participate in tasks to correct concerning, unhealthy, or high-risk living conditions including housekeeping concerns, health/hygiene, and nutrition concerns.

Home Care Services

  • Provide a trauma-informed approach to serving the community.
  • Assist residents with basic living activities, personal care, hygiene, and medication support (i.e., helping organize medications for effective use).
  • Create a safe and effective living environment for residents.
  • Log all actions, incidents, rule enforcements and report unusual or sensitive matters to Property and Program Managers.
  • Promote and teach healthy practices such as nutrition, housekeeping, personal hygiene, and laundry care.
  • Assess the need for and the cleaning of units.
    • Clean units to maintain a healthy living space for residents and the protection of the unit condition itself.
    • Assess and coordinate with Case Manager for Care Giving assessments.
    • Work with Property Manager and program team for jobs requiring a vendor, such pest removal.
  • Provide light home maintenance such as changing light bulbs, batteries in smoke detectors, unclogging toilets, etc.
  • Work in conjunction with Case Managers to meet the needs of the shared residents.
  • Coordinate with caregivers and Case Managers to meet needs of residents.
  • Follow building safety and emergency procedures according to Compass policies.
  • Contact appropriate staff for assistance and emergency services (911, police, fire department, etc.) as appropriate.

Client Services

  • Mediate conflicts and model appropriate problem solving.
  • Respond to resident issues as appropriate or as directed by the Property and/or Program Manager.
  • Report resident and/or building needs to management. Ensure supply/equipment is in stock for use.
