Position Summary:

Full-time position available in Aberdeen. The IMHT will also be working at the Ocean Shores Medical Clinic on a regular basis. Be at the forefront of a changing paradigm within health care delivery!  Full-time position on this interdisciplinary team; Responsibilities include assessment, brief intervention, and consultations with primary care, patient-centered health action plans, care coordination and care transitions with adults, youth and families.  You will be part of a team that will work toward better health outcomes by applying evidence based practices.  This is a dynamic program oriented toward health care transformation.
